<p>Beginning today, I have decided that I will be posting three times a week instead of daily.</p>
<p>As it can be guessed easily, time is the major constraints. This blog has just started to pick up its speed and I feel happy writing on this.</p>
<p>But there are other engagements too.</p>
<p>I do not want to write few lines and then post it for sake of publishing. I have chosen this blog to be a ground for sharing everything I feel and like/dislike.</p>
<p>I do not want t just wind it up everyday.</p>
<p>As this blog is not bounded by any limits of niche/audience, there is plenty to say.<span id="more-92"></span></p>
<p>But the time is factor.</p>
<p>I can post daily for few days but I can see a very busy 2010 where it may not be possible to write on this blog everyday.</p>
<p>I already write on daily basis on the two other sites of mine.</p>
<p>One is <a href="http://homeforprofits.com">Home For Profits</a>, a website about internet marketing. This was my first website that I started in 2004.</p>
<p>Another is <a href="http://boneandspine.com">Bone and Spine Health</a> which is a website about musculoskeletal health.</p>
<p>I have to write on these sites daily because I have been doing that in the past.</p>
<p>So setting a daily frequency for this website is not  practical and would be counterproductive in the end.</p>
<p>I have decide Tuesdays, Thursdays and Saturdays as probable days on which I would be posting on this site.</p>
<p>I will let you know.</p>

I wish could be furious but that does not serve any purpose. Anybody would be if one has to spend 40 minutes on a railway crossing.</p>

<p style="text-align: left;">That delayed my journey by probably more than the period  I than I waited at the crossing because it took another 30 minutes to clear the backlog and before traffic was smooth again.<span id="more-83"></span></p>
<p>I have always despised railway crossings. I do not think anyone likes them except children who love to see a moving train.</p>
<p>Railway crossings stop the movement.</p>
<p>Everyday thoousands of people stop to let a train pass. I cannot even count how many manhours are wasted at these crossings.</p>
<p>There are people who face this menace everyday.</p>
<p>Railway overbridge is a beautiful solution. Everyone including rail can move.</p>
<p>But we are still left with so many crossings that it stops making sense when we say that we are developing.</p>

<p>It is a pleasure to drive on these highways. You just zoom past everything. Flyovers have added their share to please the vehicles.</p>

<p>The nightmares are roads of city.</p>
<p>Neglected, abused and overlooked they have become weakest links in a journey between two cities.<!--more--></p>
<p>Mixed traffic, reduced road space due to encroachment by shops, potholes, manholes, ditches, cows, rickshaws, railway crossings.</p>
<p>You name an inconvenience and you would soon encounter it.</p>
<p>So the time gained on highways is eaten by the city roads.</p>
<p>I do not know when they would be improved.</p>

It was not until my mid twenties that  I realized my passion for writing. It started with poetry in Hindi, my mother tongue.</p>
<p>English has been an alien language since beginning but Hindi was natural. So I wrote in what cam naturally.</p>
<p>From Hindi poetry I then moved to verse in Hindi. I also served as Co-editor of our college magazine &#8220;Spandan&#8221;, a yearly magazine by students of Maulana Azad Medical College [The place where I did my MBBS from]<span id="more-68"></span></p>
<p>When pulse polio was introduced, I had the privilege to pen down few slogans and lyrics for promotional song.</p>
<p>After my graduation, I moved into postgraduation in orthopedics and did not write for sometime.</p>
<p>But in end of that post graduation I again wrote more than 100 poems and ghazals. I still have those with me. The previous ones had been lost due to a foolish act of frenzy.</p>
<p>Anyways!</p>
<p>Orthopedics brought out scientific writer in me and quest for spirituality. May be trauma and morbidity surrounding was responsible to ask the questions which science could not answer.</p>
<p>Thus began a journey full of pains and pleasures. Not getting an answer is very painful. And nothing is more pleasurable than realization of world and mysteries surrounding that.</p>
<p>You do not get all the answers but you do get signs that indicate you would find them.</p>
<p>In 2204 October, I founded my first website <a href="http://homeforprofits.com">http://homeforprofits.com</a>. How and why is a big question. I would write in detail one day.</p>
<p>For now it is sufficient that, it was born out of hobby and later developed into passion.</p>
<p>Further down, I founded <a href="http://boneandspine.com">Bone and Spine Health</a>, a website on orthopedics meant for people other than orthopedists.</p>
<p>Both of my sites are quite different in their audience. While one is on health, other is on internet business.</p>
<p>I like them both and they are quite successful by their own unique metrics.</p>
<p>I try writing daily for both of them.</p>
<p>There are 7 other sites that I run. But they are yet to come into mainstream called success.</p>
<p>Each of these sites have an interst or skill or hoby that dominates the content.</p>
<p>I still needed some place to write on topics unrestricted by the limits.</p>
<p>That is how I founded arunpalsingh.com</p>
<p>There are no targets to meet here, there are no forces that limit my content.</p>
<p>Rather than for any audience, it is for my selfish self.</p>
<p>But somehow deep down I feel that it is going to surpass the success of everything so far I have done.</p>
<p>Let us see.</p>

But now majority of keyboard users are novices like me ho look at keyboard when they type. [I have again pledged to learn typing by joining typing course but I know I am not going to do it before arrival of next year]<span id="more-53"></span></p>
<p>So while I type like a novice, I often press CAPS Lock button to find few words later a yelling phrase or sentence standing and looking at me in all CAPS.</p>
<p>There could be simple solution of putting it on topmost row near escape or something like that.</p>

We are trained on disease diagnosis that begins from history and then clinical examination followed by required investigations.</p>
<p>I find it very painful when someone in the need of investigation refuses because she cannot afford it.</p>
<p>What do you do?</p>
<p>Investigations are costly. The investigations require equipments and gadgets that come at phenomenal cost.</p>
<p>It is a smooth work up when one nods yes and get investigated.</p>
<p>But the problem is for not so privileged ones.</p>
<p>This becomes a difficult situation. <span id="more-42"></span></p>
<p>You cannot treat a patient unless you diagnose her.</p>
<p>And you cannot get the requisite investigation because she cannot afford the costs.</p>
<p>In India, the health system follows (To say at least) model of Social Medicine.</p>
<p>Everyone can be treated freely. But the very concept of free treatment has resulted in its failure.</p>
<p>So called state sponsored hospitals are not chosen by many people because of the quality of services they offer.</p>
<p>I am not talking about competence of the doctors in this system. I am talking about the quality of the system.</p>
<p>Poor infrastructure, patient overload and lack of equipments.</p>
<p>Many a investigations are not available in the hospital. Those available are compromised because of  overload.</p>
<p>Modren day medicine has become very dependent upon the investigations. thanks to many laws that put a doctor at the receiving end. Litigations, consumer courts and social hostility makes one practice a defensive medicine.</p>
<p>So one orders investigation for documentary confirmation inspite of clinical judgement, just to be safe. This happens in quite number of cases.</p>
<p>This trend has really hurt those who cannot afford the investigation. Included are those in which investigation is a must.</p>
<p>There seems no end to this problem. And this is not limited to investigations only. Even the cost of treatment can be a huge burden that patient or family cannot afford to take.</p>
<p>I personally feel that all people must get the best treatment that science can afford to give them. But instead the it gets delivered to only those who can afford it.</p>
<p>How many patients that who need a hip replacement would get it done.</p>
<p>Very few.</p>
<p>Rest continue to bear the pain and decide to stay as they are. And this is just one example.</p>
<p>This causes a dismay but what to do. You can only offer the treatment.</p>
